Assessment of Care
Assessment of care is a structure we use to formulate a care plan; a care assessor will visit a service user and ask a number of important, sometimes personal, questions, (we encourage input from relatives, service user representatives and carers). The care assessor will then fill in the assessment form which helps determine an understanding of the service user’s care requirements. If the service user agrees with the content of the assessment they will be asked to sign the form.
Health and Safety Risk Assessment
A risk assessment must be carried out in and around a service user’s home to minimise risks to the service user or care worker before any care commences; input from the service user, their relatives, representative and carers is always valuable, and if necessary we make suggestions as to how risks can be minimised. The service user will be asked to read the assessment and if they agree with the content they will be asked to sign the form.
